VERIFICATION FORMS MUST BE RECEIVED BY APRIL 27, 2024 TO AVOID A LATE FEE. VERIFICATION FORMS WILL BE ACCEPTED FROM APRIL 28 – MAY 11 WITH A $25.00 LATE FEE. ANY VERIFICATION FORM RECEIVED AFTER MAY 11, 2024 WILL BE ASSESSED A $100.00 LATE FEE. NO

USED TO DETERMINE IF YOUR FORM IS ON TIME, A LATE FEE ASSESSED OR IF THE FORM CAN BE ACCEPTED.

A VACCINATION RECORD MUST BE ATTACHED TO THIS FORM FOR EACH DOG LISTED.

Each handler may verify a maximum of 3 dogs, but may only show two dogs. A color photo of each verified dog is required.
