USED TO DETERMINE IF YOUR FORM IS ON TIME, A LATE FEE ASSESSED OR IF THE FORM CAN BE ACCEPTED.

A VACCINATION RECORD MUST BE ATTACHED TO THIS FORM FOR EACH DOG LISTED.

Each handler may verify a maximum of 3 dogs, but may only show two dogs. A color photo of each verified dog is required.
